The application process is fairly easy and quick. After you have completed the referral a Family Partner or Head Start Staff member will reach out to complete an application. 

*We are required to complete a full application in person with the family*

Here is a list of the primary supporting documents & information required to complete the application process.

There may be other documents or forms needed depending on what school district you reside in.

• Income Verification for 1 year; if you receive 3Squares bring you card with you.
• Social Security Numbers & Date of Birth for all members of the household
• Child’s Insurance ID Number
• Birth Certificate
• 2 Proofs of Residency
